The Whitley County Immunization Nurse will be coming to our school to give flu shots to students with a consent form fill out by a parent or guardian.  The flu shots will be given during the day on October 30th.

Please print off the attached consent form, fill out the front and back, then turn it into the school.  If you need a printed copy sent home with your child, please contact Debra Uecker through Parent Square or call the School Nurse's clinic at 260-469-5686 or 260-693-9604.
